The short answer
Cultured pearls are real pearls grown in a real mollusc with human help - not imitations. They are Mohs 2.5-4.5, organic and porous, which makes them the most chemically vulnerable material in jewelry: perfume, hairspray and cleaning dips all attack nacre.

The three categories
The FTC guides define these precisely, because the distinction is exactly where deception historically happened.
- Natural pearls form in a mollusc without human intervention. They are genuinely rare and priced accordingly - almost nothing you will see in a marketplace listing is a natural pearl.
- Cultured pearls form in a real mollusc after a nucleus is deliberately introduced. Everything about them is real pearl material; the trigger was human. This is essentially all pearl jewelry sold today.
- Imitation pearls are glass or plastic beads coated to look like pearl. They are not pearls, and the guides require them to be described as imitation or simulated.
“Cultured” is not a hedge and does not mean fake. A cultured pearl is a pearl.

Almost all affordable pearl jewelry is freshwater. Freshwater pearls are less perfectly round than akoya and vary more in surface, which is a difference in character rather than in authenticity - and it is why they cost a fraction as much.
Telling real from imitation
The tooth test. Rub the pearl gently against a tooth edge. Real nacre feels slightly gritty; glass and plastic feel perfectly smooth. This is the oldest test and it works.
Look at the drill hole. Real pearls show layered nacre at the hole. Imitations often show a coating flaking away from a bead core.
Look for variation. Real pearls on a strand differ slightly in shape, size and surface. Perfectly identical pearls are usually manufactured ones.
Temperature. Real pearls feel cool at first touch and warm slowly. Plastic is room temperature immediately.
Care, which is the actual subject
Pearls are calcium carbonate laid down in layers with an organic binder. That structure is beautiful and chemically fragile.
- On last, off first. Perfume, hairspray, cosmetics and even hairspray overspray all attack nacre. This single habit does more than everything else combined.
- Never in an ultrasonic cleaner or a jewelry dip. Both destroy pearls. This is not a caution, it is a prohibition.
- Wipe with a soft damp cloth after wearing to remove skin oils and sweat, which are mildly acidic.
- Store flat and separately, not coiled and not loose with harder jewelry, which will scratch them.
- Restring strands periodically. Silk stretches and dirties. This is normal maintenance, not a fault.
- Do not store them airtight for years. Pearls contain water and very dry storage can dull them.

Common questions
Are cultured pearls real pearls?
Yes. A cultured pearl grows inside a real mollusc and is made of real nacre; the only human involvement is introducing the nucleus. That is different in kind from an imitation, which is a coated bead.
How can you tell if pearls are real?
The tooth test is the most reliable quick check - real nacre feels gritty, imitations feel smooth. Also look for slight variation between pearls on a strand and layered nacre at the drill hole.
Can pearls get wet?
Brief contact with fresh water is fine and wiping them with a damp cloth is recommended. Swimming is not - chlorine and salt attack nacre, and water in the drill hole rots the thread.
How often do pearl necklaces need restringing?
It depends on how often they are worn; frequently-worn strands need it more often than occasional ones. Signs to look for are stretching between pearls and grubby thread at the knots. Treat it as normal maintenance rather than a repair.
