What a flat back is

Instead of a post with a pushed-on butterfly clutch, a flat back earring has a smooth flat disc that sits against the back of the lobe, with the decorative end attaching on the front. The two halves connect by a fine internal thread or by a threadless friction fit.

It is the style piercers use, and it is used for a mechanical reason rather than an aesthetic one.

Why it fixes irritation butterflies cause

Threaded and threadless
