What the grades mean
Titanium jewelry uses a few designations and they are not interchangeable:
- ASTM F136 / Ti-6Al-4V ELI / Grade 23 / G23 - the surgical implant alloy. ELI stands for extra low interstitial, meaning tighter control of impurities. This is what you want.
- Grade 1 / Grade 2 (commercially pure titanium) - also nickel-free and perfectly safe, softer than G23. Sometimes described as ASTM F67.
- “Titanium steel” - not titanium. This is stainless steel with a misleading name, and it contains nickel.
- “Titanium plated” - a coating over something else. The something else is not disclosed.
The first two are fine. The last two are the exact traps this site exists to flag, and they appear constantly in earring listings aimed at sensitive ears.

Buying an 18G post for a piercing that has settled at 20G means forcing a thicker bar through a healed channel - which irritates the piercing and can set healing back. If you are not sure what gauge you are, 20G is the safer purchase; it fits loosely in an 18G hole rather than not fitting at all.
